How long have you been in business?
Lifescape Community Services, Inc. has been serving the senior population since 1999. The agency was created with the merger of Meet and Eat, which offered home delivered and congregate meals, and Cornerstone Resources which operated Brightside Adult Day Service and provided Information and Assistance.
What is your primary product or service?
The primary programs Lifescape offers include Nutrition Services, which administers the Meals on Wheels program, 18 congregate dining sites in Winnebago, Boone, Ogle and Lee counties and lunches for many private day care operations in Rockford. Brightside Adult Day Service provides a safe and supervised place for those with disabilities can go during the day time hours to provide their care givers with rest and lessened worries. Progam Services provides our clients with help in filing for the many local, state and federal programs for which they may be eligible. Some of these programs include services through the City of Rockford Department of Human Services, Illinois Cares RX and Circuit Breaker program and applying for Medicare Part D.
How many locations do you have and do you have plans to expand?
Nutrition Services operates our main kitchen at 705 Kilburn Avenue and a second in Oregon, Illinois, for dining sites in Ogle and Lee County. Program Services operates out of our 705 Kilburn Avenue building. Brightside Adult Day Service has facilities at 1055 East State Street and at 2233 12th Avenue, both in Rockford

Lifescape Community Services, Inc. is a 501(c)(3) non-profit which is governed by an executive director and a board of directors.
What are your hours of operation?
The main office is open from 7:30 a.m. t0 4:00 p.m. Monday through Friday.
